General Booking Forms:
Caribbean Explorer I Forms - Bahamas
- Trip Information (47 kb) - Detailed information about the itinerary and trips offered aboard the Caribbean Explorer I in the Bahamas.
- Nitrox Waiver (16 kb) - required to be signed on board prior to diving with nitrox. Copies are available on board as well; we do not need to receive this form in the booking office prior to your trip
- Caribbean Application & Liability Release Form - To be filled out and sent to your booking office to confirm your reservation. The original of the Liability Release Form MUST be given to the captain upon boarding the vessel.
- Reservation Form - US Sales Office - to be filled out and faxed or mailed to our US office in order to confirm a reservation made with that office
Caribbean Explorer II Forms - Saba, St. Kitts, Statia, St. Maarten
- Trip Information (47 kb) - Detailed information about the itinerary and trips offered aboard the Caribbean Explorer II.
